2013 Miss New Mexico Contestant Packet and Forms

FORMS FOR A CONTESTANT TO APPLY TO A LOCAL COMPETITION ARE:




MISS STATE PAPERWORK PACKET FOR ALL LOCAL WINNERS
QUALIFIED TO ATTEND THE 2013 STATE COMPETITION. 

 

SPECIAL INSTRUCTIONS

Special Instructions: Fill out these papers as if you were the state winner for 2013. Ignore the national due dates and the national person to send forms to. All forms will come to Carol Henry at carolH@pvtnetworks.net or to jjklump@tularosa.net.

The due date for all state forms will be April 30. You must have all forms submitted either by email or snail mail by April 30! Once again, do not follow the national due dates and do not send any of these forms to the national offices as requested.

The reason we are doing this, these are the national forms that the new Miss NM must submit immediately following her crowning. Since the national pageant will be help in September this year, all paperwork will be due immediately. By using these forms for the state competition, the newly crowned winner can focus on other things her first week.

The following items are the forms you must fill out:
1. CONTRACT. Your local director should have your copy unless you need to re-type it nicely
2. FINALIST TALENT REQUEST FORM
3. TALENT INTRO- in 25 words or less, write your talent introduction for the emcee to read.

Form 6 - Photo Requirements....note that the requirement is different for the state this year. It is the same as the National requirements.

Form 7 - Photographer Agreement...you only use one of these. It depends on your photographers status. If he is a corporation, use that one. If your Mom takes the photo, use the individual. We will NOT reproduce your photo in any print materials unless we have this form signed and notorized from your photographer, no matter who they are! That means you could be left out of the program book, newspaper advertising, etc.

Ignore the next 5 pages which pertain to National Participation. Read the material so that if you are Miss NM 2013, you will be familiar with it.

Form A-1 Talent music reservation-this form assures you the exclusive right to your song if reserved before another girl.

Form B - wardrobe questionaire

Form C -talent rehearsal consent...you may have your talent coach present in the auditorium for your talent rehearsal. We will give her 5 minutes to consult with you following your first talent rehearsal, then she may watch you as you go through the talent the second time. Unless she is badged in, she must then leave the auditorium for the rest of the rehearsal.

Form A-2 - Detailed Talent Music Form... must be filled out completely. Call Carol if you don't understand how to do the BMI/ASCAP/SESAC search for your song or if you have other questions.

Form E - must be filed if someone did the music arrangement for your music track (most of you will skip this)

Once again, do not read this check list and get confused as to when and who to send your state forms to. THEY DO NOT GO TO THE NATIONAL OFFICE ON THE DATES THEY ARE REQUESTING!

SUMMARY OF PACKET CONTENTS

National Contestant’s Checklist:
The Contestant Checklist of the Administration and Production items that are required by the Miss America National Office and their respective due dates. Please use this form as your reference and easy reminder of items required by MAO. A similar checklist is used at the MAO National Office to track and record the received items required from the Contestant. A copy of this checklist will be emailed to the State Executive Director and the Contestant acknowledging items received. The items listed under the State Executive Director Requirements heading are those that the State Executive Director will provide to the Miss America Organization.

Due Dates:
the due date for each required item is referenced on the Administration & Production Requirements Checklist. Please be sure to adhere to these due dates—it is important.

Contestants and Parents Mailing Information:
Contact information for Parents and Contestants. Please be sure to include mailing addresses, email addresses and phone numbers along with the shipping address for items sent to the Contestant prior to the National Finals. If any of this contact information changes, please notify us immediately.

National Contestant Contract:
After your Contestant signs, forward two (2) copies, properly notarized on pages 14 or 15, along with all attachments to Mary Ellen Lucia in Field Operations of the Miss America Organization. The National Contestant Contract is available on the MAO Resource Portal which can be accessed by the State Executive Director.

A copy of the Contestant’s Birth Certificate, Driver’s License, Medical Insurance Card, Official College Transcript and/or copies of College Degrees awarded should also accompany the Contestant Contract. Please plan ahead to have these items ready to mail with the National Contestant Contract. The contract is not complete unless these items are included. Please do not fax as the information is not always legible when received in this format. These items need to be mailed hardcopy in one complete packet. Once approved and signed by MAO’s CEO, one copy of the contract will be returned to you for your files.

National Contestant’s Platform Statement:
Follow the preformatted page using the instructions exactly as written. A Sample Contestant Platform Statement is included as a reference. Important: The Platform Statement must be signed and dated in the lower right hand corner by the Contestant in order for it to be accepted by MAO.

National Contestant’s Resume:
Follow the preformatted page using the instructions exactly as written. A Sample Contestant Resume is included as a reference. Do not sign the Resume. The signed Platform Statement covers the information listed for both the Contestant Platform and Resume. Detailed instructions are included in the Instructions for Typing the National Contestant’s Platform Statement and the Instructions for Completing the National Contestant’s Resume.

National Contestant’s Media Questionnaire:
Specific questions used for the Miss America Competition Magazine and broadcast media.

Contract Free Agreement:
Details MAO’s policy regarding advertising, photographs and publicity prior to the National Finals. This document must be signed by the Contestant and the State Executive Director.

Headshot and Candid Photo Requirements:
A list of the headshot photographs required for the National Competition Magazine, to post on the MAO website, and in other broadcast media along with the information on submitting the candid photos with their individual descriptive captions.

Copyright Assignment Agreements-Individual Photographer & Photography Corporation:
Signed by your photographer. Agreements must be notarized. This agreement is vital. We cannot use contestant photographs in any media format until this agreement is received.
